$450 per year plus HST ($508.50 total). That works out to about $42 per month. There is also a monthly payment option at $50/month with a 12-month commitment. For a private family practice in Toronto, this is a fraction of what concierge clinics charge.
Yes. You can book a single pay-per-visit appointment for $100. No membership required. Pay-per-visit appointments do not include lab services, telemedicine, or certain services that require an ongoing clinical relationship. Those are available to members. If you decide to join afterward, you can sign up anytime.

Yes. Your NP can refer you to specialists, order imaging, and coordinate your care with other providers. They handle the full referral process so you do not have to manage it yourself.
Your NP can send your records to the emergency department, coordinate with hospital staff, and follow up with you after discharge. If you need a specialist, your NP handles the referral. You are not on your own.
For the vast majority of family practice needs, your NP handles everything: chronic disease management, preventive care, mental health support, treatments, referrals, and lab work. In a primary care setting like Care&, NPs do not perform surgical procedures or admit patients to hospital. For anything outside their scope, your NP will coordinate a referral to the right specialist.
Yes. Both locations have on-site labs. Members can get bloodwork done during or after their appointment without a separate trip. Pay-per-visit patients can receive a lab requisition for an OHIP-covered external lab.
