School, Camp & Sports Forms

Every year it's the same scramble: school needs a health form, camp needs a medical, your kid needs a sports physical. At Care&, these are included with membership. Book a visit, bring the form, and your NP takes care of it. No extra fees and no chasing down a walk-in clinic.

A Family-Friendly Approach

Kids aren't small adults. They need a provider who's patient, knows how to talk to them, and doesn't rush through the visit. Our NPs take their time, explain things in language kids can understand, and make the clinic feel as comfortable as possible.

Parents get the full picture too. Your NP discusses what's going on, what to watch for at home, and when to come back. No guessing, no Googling.

When to Go to the Emergency Room

Care& is not an emergency or urgent care facility. If your child shows any of the following, call 911 or go to your nearest emergency department immediately:

  • Difficulty breathing or rapid breathing
  • Seizure or loss of consciousness
  • Fever above 38°C (100.4°F) in infants under 3 months
  • Severe allergic reaction (swelling of face/throat, difficulty swallowing)
  • Uncontrolled bleeding or a head injury with vomiting
  • Signs of dehydration (no wet diapers for 6+ hours, no tears when crying)
  • Stiff neck with fever and sensitivity to light
  • Sudden severe abdominal pain

For non-emergency concerns outside clinic hours, send a message through the app and your NP will follow up the next business day.

We see patients of all ages, from newborns to seniors. Our NPs have training in pediatric care and are comfortable managing children's health from infancy through adolescence.
Yes. We complete school enrollment health forms, camp medical forms, sports physicals, and immunization records. These are included with your child's membership.
Yes. We follow the publicly funded Ontario immunization schedule. Vaccines on the provincial schedule (like MMR, DTaP, meningococcal, and HPV) are covered by OHIP and administered at no charge. Some vaccines not on the provincial schedule — such as Hepatitis A, travel vaccines, and certain optional immunizations — are privately paid and priced separately. Your NP can walk you through what applies to your child.
Yes. Many families have all members, parents and children, seeing the same NP. This gives your NP a complete picture of your family's health. Each family member needs their own membership.
Our clinic is open Monday to Friday, 8am to 5pm. For urgent concerns outside these hours, go to your nearest emergency room or call 911. You can also send a message through the app and your NP will follow up the next business day.
Our NPs can assess for ADHD and manage treatment including medication. For complex cases or if a formal psychological assessment is needed, we can refer to a pediatric specialist.
Each child needs their own Family Practice membership at $450+HST per year. This includes unlimited visits, vaccinations, well-child checks, sick visits, and school forms.
