How It Works

Create Your Account
Register at app.careand.ca in under two minutes. You'll enter your basic info, health history, and emergency contacts. Everything is stored securely and encrypted.
Choose Your Plan
Pick the option that fits your needs. Family Practice membership is $450+HST/year (or $50+HST/month on a one-year plan) for unlimited visits and a dedicated NP. Or book a single visit ($100) if you just need one-time care.
Book Your First Appointment
Select your preferred NP, location (Yorkville or Lawrence Park), and time. Most new members are seen within a few days. Virtual visits are also available.

The average OHIP family doctor appointment lasts 7 minutes. Your first Care& visit is 30 minutes — and follow-ups are 15-30 minutes. Enough time for a real conversation.
Longer Appointments
Your first visit is 30-45 minutes. Enough time for a proper assessment and a real conversation about your health.
No Rushing
Your appointment gets the time it deserves. No rushing.
Health History Review
Your NP will go through your medical history, current medications, and any concerns. If you have old records, bring them along or upload them through the app.
A Care Plan
You'll leave with a clear picture of next steps. Whether that's lab work, a referral, medication, or a follow-up.
What to Bring to Your First Appointment
Having these ready makes your first visit faster and more useful
Health Card (if you have one)
Your Ontario health card helps us verify your OHIP number for lab requisitions. No health card? No problem — Care& doesn't require one.
Medication List
A list of everything you're currently taking — prescriptions, over-the-counter medications, vitamins, and supplements. Include dosages if you know them.
Previous Medical Records
Any recent lab results, specialist reports, imaging, or discharge summaries. You can bring physical copies or upload them through the app before your visit.
Questions and Concerns
Your first appointment is 30 minutes. Write down anything you want to cover so you don't forget. There's no rushing here.
