Your NP reviews your medications regularly. Are they working? Any side effects? Could a different combination work better? We don't just set it and forget it.

Lab Tracking

On-site lab work means your NP can order and review your blood work in one visit. Track your A1C, cholesterol, thyroid levels, and kidney function over time without extra trips to a separate lab.

Lifestyle Coaching

On-Site Lab Work

Chronic disease management requires regular blood work. At most clinics, that means a separate trip to a lab, waiting in line, and coming back another day for results. At Care&, the lab is in the clinic. Get your blood drawn right after your appointment, and your NP reviews the results in context with everything else they know about your health.

All lab work is included with your Family Practice membership. No extra fees, no separate billing.

Digital Health Tracking

The Care& app gives you access to your lab results, visit notes, and prescriptions in real time. You can also connect Apple Health to share data from home monitoring devices like blood pressure cuffs, glucose monitors, and fitness trackers. Your NP sees the full picture, not just what happens during your appointment.

Yes. NPs in Ontario are authorized to diagnose, prescribe, and manage chronic conditions independently. Our NPs manage diabetes, hypertension, thyroid disorders, asthma, and more every day.
It depends on your condition and how stable it is. New diagnoses or medication changes might mean monthly visits. Once things are well-controlled, quarterly or twice-yearly check-ins are typical. With membership, all visits are included.
Yes. NPs can prescribe all diabetes medications including insulin, GLP-1 agonists, SGLT2 inhibitors, and metformin. We also help with dose adjustments and monitor for side effects.
Yes. Blood work done at our on-site lab is included with your Family Practice membership. This covers routine monitoring labs for chronic conditions like A1C, lipid panels, thyroid function, and kidney function tests.
Your NP can refer you to endocrinologists, cardiologists, respirologists, rheumatologists, and other specialists. Having a primary care provider who coordinates your care makes the specialist process smoother.

Phone and video visits are included with membership. If you have a quick question about a reading or a symptom between visits, you can reach your NP without coming into the clinic.
