What You Bring:
- Authorized to work in Canada.
- Master of Nursing or equivalent.
- Completion of a Nurse Practitioner Certificate or equivalent.
- Current registration (Extended Class) in good standing with the College of Nurses of Ontario.
- Current liability insurance.

Registered Practical Nurse (RPN)
Location: Toronto (Yorkville & Lawrence Park)
Role Overview:
The full-time Registered Practical Nurse (RPN), reporting to the Clinical Director, will be integral to our patient care team, working under Nurse Practitioner medical directives. This role is key in conducting health assessments, managing the phlebotomy department, and utilizing our innovative, technology-driven approach to patient care for all age groups.
As an RPN at Care&, you will:
- Manage phlebotomy services, including venipuncture, sample procurement, specimen transport, and liaising with external labs for processing and results.
- Conduct subjective health assessments and objective physical examinations of patients of all ages.
- Perform specific acts and health teaching under the direction of the nurse practitioner, including immunization, various injections, wound care, cryotherapy, point of care testing, and STI testing.
- Utilize your understanding of immunization administration, transcription, and immunization form reconciliation (i.e., school/employment forms).
- Work with our AI-integrated EMR system that streamlines documentation and clinical workflows.
- Respond to general patient enquiries and help educate patients as per nurse practitioner’s directives.
- Arrange appropriate follow-up, referrals, and investigations according to established directives or instructions.
- Implement nurse practitioners’ orders, ensuring patients undertake requisite diagnostic testing and medication administration, and educate patients on the purpose and preparation for such tests.
- Communicate test results to patients and external providers as directed by nurse practitioners.
- Ensure incoming urgent medical results are brought to the attention of the treating nurse practitioner in an expedient manner.
- Triage patient phone calls that require a level of medical expertise beyond the medical administrator’s scope.
- Order and monitor inventory of vaccines, drug samples, medical equipment/supplies, and emergency medical supplies as required.
- Refresh and prepare patient examination rooms before and after patient encounters as required.
- Ensure the regular inspection, testing, calibration, and maintenance of primary health care equipment is completed annually or as required.
- Participate in chart and case reviews, the development of policies and procedures, and support patient education.
- Collaborate effectively as a team member with all staff at Care& Family Health, including front office and clinicians, for optimal patient care.
- Routinely participate in staff meetings and continuing medical education.

What You Bring:
- Current registration with the College of Nurses of Ontario as a Registered Practical Nurse and member in good standing.
- 2+ years of nursing experience working in a primary healthcare setting. (New graduates with paediatric placement experience will also be considered).
- Phlebotomy certification.
- Up-to-date CPR certification.
- A clear police check.
- Up-to-date infectious and communicable disease vaccinations.
- Proficiency in English, both written and verbal.
- Thorough knowledge and understanding of medical/laboratory procedures, charting, and current health and safety practices.
- Experience working with children, youth, and families within a culturally diverse community.
- Commitment to providing safe, competent, and ethical nursing care.
