General Questions

Care& is a modern family practice clinic in Toronto led by experienced Nurse Practitioners. We offer a membership-based Family Practice model that gives you unlimited access to the healthcare services you need.

Our approach combines the best of traditional family medicine with modern technology, offering fast access to appointments, telemedicine, on-site lab testing, and direct access to your care team. See our full list of services.

No, Care& services are not covered by OHIP. We operate as a private pay clinic offering Family Practice memberships at $450 + HST per year.

While our services aren't covered by provincial health insurance, many extended health insurance plans offer reimbursement for Nurse Practitioner visits and private healthcare services. Check your insurance coverage.

We have two convenient locations in Toronto:

Family Practice members can visit either location and receive care from any of our Nurse Practitioners.

Both clinic locations are open Monday through Friday, 8:00 AM to 5:00 PM.

Phone and video consultations are available during these hours. We're closed on weekends and statutory holidays.

No, Care& is not a walk-in clinic. All visits require appointments to ensure you receive unrushed, high-quality care.

However, we offer same-day appointments for urgent concerns and can often accommodate last-minute scheduling needs for our Family Practice members.

Yes, we provide pediatric care for children of all ages, from newborns through adolescence.

Our services include well-child visits, immunizations, sick visits, developmental assessments, and management of chronic conditions. Each family member needs their own membership — we see all ages, so the whole family can be with the same NP at the same clinic.

Pricing & Membership

Family Practice membership is $450 + HST per person per year ($508.50 total).

This provides unlimited in-person appointments, unlimited phone and video visits, on-site lab testing, prescription refills, and access to both clinic locations. See full pricing details.

Yes. Each family member needs their own membership at $450+HST per year. We see patients of all ages, from newborns to seniors, so your whole family can be with the same NP at the same clinic.

Yes, we offer Pay-Per-Visit appointments at $100 each.

However, Pay-Per-Visit does not include lab testing, prescription refills, telemedicine, or access to controlled substances. If you need ongoing care, a Family Practice membership provides significantly better value.

We accept credit cards, debit cards, and electronic bank transfers. Payments are processed securely through our member portal.

Family Practice memberships can be paid annually or set up on automatic monthly billing.

Many extended health insurance plans provide coverage for Nurse Practitioner visits and private healthcare services.

We provide detailed receipts that you can submit to your insurance provider. Learn more about insurance coverage.

For Pay-Per-Visit patients, we require at least 4 hours notice to cancel or reschedule. Appointments cancelled with less than 4 hours notice or no-shows may be charged a fee.

Family Practice members have more flexible cancellation. If you need to reschedule, please do so as early as possible through the app so the slot can go to another patient.

You'll receive login credentials after your first appointment. The portal works on any device with a web browser and is also available as a mobile app.

The member portal allows you to:

  • Book, reschedule, and cancel appointments
  • Request prescription refills with one click
  • View your health records and test results

  • Complete intake forms before visits
  • Access your immunization records
  • Pay invoices and manage billing

Yes. We use bank-level encryption and comply with all PHIPA and PIPEDA privacy regulations.

Your health information is stored securely in Canada, and we never share your data without your explicit consent. Our platform undergoes regular security audits.

Yes, we can request and integrate records from your previous healthcare providers with your consent.

You can also upload documents directly through the member portal, including lab results, specialist reports, and imaging studies.

Yes, Family Practice members have unlimited access to phone and video consultations.

Video visits are conducted through our secure member portal and don't require any additional software or downloads.

Yes, our member portal is available as a mobile app for iOS and Android devices.

The app provides full access to all portal features, including booking appointments, requesting refills, and viewing your health records on the go. Learn more about the Care& app.

If you've forgotten your password, use the "Forgot Password" link on the login screen at app.careand.ca. A reset link will be sent to your registered email address.

If you're not receiving the reset email, check your spam/junk folder. Still having trouble? Contact us at helpdesk@careand.ca or call (647) 951-4770 during clinic hours (Monday to Friday, 8 AM to 5 PM) and we'll get you sorted out.

Family Practice

Your Family Practice membership includes:

Family Practice members often have same-day availability for urgent concerns, though this cannot be guaranteed. Routine appointments are usually available within a few days.

Yes, continuity of care is a priority at Care&. You'll be assigned a primary Nurse Practitioner who will manage your ongoing care.

If your primary NP is unavailable, another member of our team can see you, and they'll have full access to your health records.

We provide family practice care for a wide range of conditions, including:

Yes, our Nurse Practitioners can prescribe controlled substances, but only for Family Practice members with an established care relationship.

This policy ensures safe, responsible prescribing practices and appropriate monitoring.

Yes, we provide referrals to specialists when medically necessary. Our team has established relationships with specialists across Toronto.

We'll coordinate your care, share relevant medical records, and follow up on specialist recommendations.

Of course. We'll request your medical records from your previous provider so nothing gets lost in the switch.

Your first appointment will include a full health assessment so we have a clear picture of your medical history and current needs.

Pay-Per-Visit

Pay-Per-Visit is our option for patients who need care for a specific concern but don't require ongoing family practice care.

Each visit costs $100 and includes a full assessment and treatment plan for your concern.

Pay-Per-Visit provides single-visit care for specific concerns, while Family Practice offers ongoing care with full access to your NP.

Pay-Per-Visit does NOT include lab testing, prescription refills, telemedicine access, or the ability to obtain controlled substances. Family Practice members receive all these benefits plus unlimited visits.

No, lab testing is not included with Pay-Per-Visit appointments.

If lab work is needed, we can provide a requisition for testing at an external OHIP-covered lab, or you can upgrade to a Family Practice membership for on-site testing.

No, prescription refills are not available for Pay-Per-Visit patients.

To receive ongoing medication management, including refills, you'll need to become a Family Practice member.

No, telemedicine (phone and video visits) is only available to Family Practice members.

Pay-Per-Visit is for in-person visits only.

Pay-Per-Visit is appropriate for:

  • Minor acute illnesses (cold, flu, infections)
  • Minor injuries (sprains, cuts, minor burns)
  • Skin conditions (rashes, minor wounds)
  • Single-concern assessments
  • Medical documentation (sick notes, clearance forms)

For ongoing care, chronic conditions, or anything requiring follow-up, Family Practice is recommended.

Yes, you can upgrade to Family Practice membership at any time.

Executive Medicine

Executive Medicine is our upcoming premium healthcare service for professionals who want proactive health management with maximum convenience. It is currently in development and will be launching soon.

It will include everything in Family Practice plus extended appointments, a thorough annual health assessment, and priority access to your care team. Learn more and join the waitlist.

Executive Medicine will be $1,500/year + HST. This will include everything in the Family Practice membership plus priority scheduling, extended appointments (45-60 min), direct email access to your NP, and an annual executive health assessment.

Executive Medicine members have direct email access to their Nurse Practitioner during clinic hours (Mon-Fri, 8am-5pm).

For urgent concerns outside clinic hours, go to your nearest emergency room or call 911. You can also send a message through the portal and your NP will follow up the next business day.

Yes, we offer family plans for Executive Medicine with customized services for each family member.

Corporate Health

Corporate Health is our employer-sponsored healthcare program that provides family practice services as a workplace benefit.

IUD Insertions

Yes, our Nurse Practitioners are trained and experienced in IUD insertion and removal.

We offer both hormonal IUDs (Mirena, Kyleena) and copper IUDs (Mona Lisa, Ballerine, Flexi-T), depending on your needs and preferences.

The process includes:

  1. Initial consultation to discuss pain management options and ensure an IUD is right for you
  2. Pre-insertion testing (STI screening if needed)
  3. IUD insertion appointment (60 minutes)
  4. Follow-up appointment to ensure proper placement

The entire process typically takes 2-3 days from consultation to insertion.

IUD insertions are free for family practice members or a $250 procedure fee for pay-per-visit patients. The IUD device itself typically costs $100-450 and may be covered by your insurance. Patient must have the IUD already prescribed and obtained before their insertion appointment.

Endometrial Biopsy

An endometrial biopsy is a procedure to collect a small sample of tissue from the lining of the uterus (endometrium) for laboratory analysis.

It's performed to investigate abnormal uterine bleeding, thickened endometrium, or other uterine concerns.

Your Nurse Practitioner may recommend an endometrial biopsy for:

  • Abnormal uterine bleeding (heavy, prolonged, or irregular periods)
  • Post-menopausal bleeding
  • Thickened endometrium seen on ultrasound
  • Evaluation of endometrial polyps
  • Investigation of potential endometrial cancer
  • Infertility workup

  • Care& Reset (a Myers cocktail for immune support, energy, general wellness)
  • IV Iron

We also offer vitamin injections (B12, Vitamin D) and glutathione.

PREP & LGBTQ+ Health

Yes, we provide full PrEP services including assessment, prescription, IM administration and ongoing monitoring.

PrEP (Pre-Exposure Prophylaxis) is a highly effective medication that reduces the risk of HIV infection by over 99% when taken as prescribed.

Starting PrEP involves:

  1. Initial consultation to assess your need and eligibility
  2. Baseline testing (HIV, kidney function, STI screening)
  3. PrEP prescription (we prescribe Truvada, Descovy, and Apretude as well as generics)
  4. Follow-up visits every 3 months for monitoring and refills

Most patients can start PrEP within 1-2 weeks of their initial consultation.

PREP medication costs approximately $250-300 per month if paying out-of-pocket.

However, PREP is covered by:
