Provider Referral Partnerships
We partner with specialty clinics across Toronto to provide their patients with high-quality family practice.
Many specialty providers don't offer primary care but need a trusted partner for their patients. Whether you run a fertility clinic, women's health practice, mental health clinic, or another specialty, Care& provides the ongoing family practice your patients need between visits to you. Your patients get their own Nurse Practitioner, on-site lab work, and full health records they can share with your team.

What Your Patients Get
Unlimited visits with a dedicated NP, phone and video consultations, on-site lab testing, prescription management, and health records accessible online. All for $450/year per patient.
How Referrals Work
Submit a referral through our online form. We'll reach out to the patient directly, get them onboarded, and keep you in the loop on anything relevant to their specialty care.
